Forms for your employee's file
The following forms must be kept in your employee's file at your designated site.
NY's Wage Theft Prevention Act requires one of the following forms to be completed annually and a copy given to the employee:
Forms to submit to your payroll specialist
Use the forms that your employee has completed above to submit the information to your specialist using the Employee Data Sheet.
If your employee chooses to enroll in direct deposit, you must submit the signed Direct Deposit Agreement.
To update your company banking information, please submit the form below to your payroll specialist.
If you have a change in address or change in entity, you must also notify the IRS and state agencies and insurance companies of your changes. Please call our office and make an appointment so start this process.
